Not illustrated. 1922 Excerpt: ...Concerning the discussion as to whether the earliest changes in pulmonary tuberculosis are Inspiratory or expiratory, we offer the following as a tentative contribution to the solution of the problem. The earliest changes in the physical signs produced by the tuberculous process Itself are In the most common sequences diminished respiration, slightly impaired resonance, broncho vesicular breathing with prolonged expiration, and, finally, fine rales. But if, as is almost invariably the case, the patient has not come under observation until after the formation of a central or mediastinal pneumothorax, the earliest signs will be those of the pneumothorax, d'Espine's sign, tracheal breathing of more than normal extent, hyperresonance or even slight tympariy. and bronchial inspiration in the adjacent area. When the infiltration has progressed still further, its own proper signs are no longer entirely suppressed by the pneumothorax and are recognized as being produced in an apparently much later stage of pathological change. Of course, in practice many different combinations of pathological changes are found, and if studied by all of the diagnostic procedures, the problem becomes too complex for discussion except in a special article. To recapitulate, the breathing over a small pneumothorax will be increased if the pneumothorax lies in contact with the trachea, with an open cavity or with any form of solid tissue which in turn lies in contact with a bronchus. It will be diminished if the pneumothorax lies over alveolar tissues only, or if there is much pleural thickening or exudate. It will be more or less amphoric if the pneumothorax is either recent or of moderate size. It will be essentially tracheal if the pneumothorax is in contact with the upper mediastinum...
